ABSTRACT
Important/potential value of macrolides has been proved in the management of chronic respiratory diseases by increasing basic and clinical trials.Through three face-to-face discussions,10 experts examined important data and drafted this consensus related to macrolides:(1) mechanism of nonantiinfective effects;(2) clinical use in chronic respiratory diseases;(3) cautions of long-term use.The mechanism out of non-antiinfective effects includes anti-inflammatory effect,modifying airway secretion,immune-regulation related to antibacterial effect,corticoid saving effect and anti-viral effect.The efficacy of long-term use of low-dose macrolides is definitely confirmed in diffuse panbronchiolitis,chronic rhinosinusitis.It is considerably used in bronchiectasia,cystic fibrosis,severe asthma and chronic obstructive pulmonary disease.Further studies should be conducted in cryptogenic organizing pneumonia and respiratory viral infection.It should be paid attention to its possible adverse effects (including drug interactions,cardiac toxicity,ototoxicity and disturbance of intestinal flora) and drug resistance in long-term use.A Chinese consensus for non-antiinfective effects and clinical use of macrolides is developed for the first time,which aims to expand their rational use and the further research.

Objective] To explore compatibility of medicines, producing and clinical application of Astringing Lung Hemostatic Paste. [Methods] Expound theoretical basis, basic prescription,basic medication and main efficacy based on its pharmaceuticals, analyze its application on syndrome heat invading the lung, lung and spleen qi deficiency,lung yin deficiency,lung and kidney deficiency,state advantage, manufacturing methods and directions, explore clincal application combined with cases.[Result] Influenced by Zhang's theory and combined with rules of treatment, restrain the dispersing and nourish the deficiency, Astringing Lung Hemostatic Paste derived from Baihe Gujin decoction, Bufei Ejiao decoction and Danxi Kexue decoction, lays emphasis on recuperation of lung, spleen, and kidney and effects on eliminating phlegm and astringing the lung and stanching bleeding and tonifying kidney. Decocting the primary medicinal herbs of 10 days into paste which meets one month-taking.[Conclusion] The paste,eliminating phlegm and astringing the lung and stanching bleeding and tonifying kidney, well effects bronchiectasia and hemoptysis, which saves medicinal materials and is convenient to take.

In this paper, we report four patients with chronic respiratory diseases who improved by administration of Bukuryo-kyonin-kanzo-to.<br>A 66-year-old female diagnosed as bronchial asthma could increase the peak flow rate and stopped western medicines by its administration. A 63-year-old female suspected to have cough variant asthma and an 08-year-old female diagnosed as having bronchiectasia with cough and dyspnea on exertion improved gradually after a few months of treatment. A 62-year-old female diagnosed as having interstitial pneumonia with systemic sclerosis showed improvement of cough and dyspnea. The erythrocyte sedimentaion rate and serum lactate dehydrogenase decreased and arterial oxygen pressure increased by its administration. This report and past reports seem to indicate that a patient profile which can be expected to be responsive to this formula is: elderly female, cough worse at night, chest dyspnea, and epigastric tenderness and muscle guarding. This drug was considered to have wider applications for chronic respiratory disease.
